PROGRAM OVERVIEW:This program is designed for those who want to improve physical and mental health, lose weight or reduce the need for medications for other conditions such as hypertension, diabetes or joint pains.Your success in the program will depend on your willingness to make changes to your nutrition and exercise habits.
APPOINTMENTS, CANCELLATIONS AND MISSED APPOINTMENTS: Appointments should be scheduled and can be re-scheduled or canceled (up to 24 hours before the scheduled appointment) through www.evolvewithjt.com.
I have a 24-hour notice for cancellation of an appointment. If cancelling on the same day of the appointment, but before the appointment, clients will be charged a fee of $100.If you are late for a session, you may lose some of that session time. No-shows will be charged the full out-of-pocket fee for their appointment.This is to ensure that access to care is not taken away from other clients and to discourage misuse of resources.The fee will be deducted from the pre-paid package amount or invoiced is necessary.
FINANCIAL POLICY & PAYMENT:
Payment is due when the services are booked.I accept Mastercard, Visa and PayPal Credit. Some HSA/FSA credit cards may be accepted for services, but it is the client’s responsibility to determine if your HSA/FSA will cover nutrition consultations. Payment plans may be offered for the 90-Day Private Coaching Program.
COMMUNICATION BETWEEN APPOINTMENTS
I will generally respond to emails or messages within 24-48 hours. If the question is more in depth or takes longer than 15 minutes to answer, I will ask that we discuss it in our next scheduled appointment. If a true emergency situation arises, please call 911 or any local emergency room.
Refunds will not be given for pre-paid plans and packages.
TERMINATION OF SERVICES
While this is rare, Coach James Thompson has the right to terminate services. In this case, a refund will be given for the remaining visits owed. As a client, you have the right to terminate services at any time and agree to give proper notification. See cancellation and refund policy.
